This client was already pursuing some advice and diet plan of for weight loss from our online dietitian for Indians living in Singapore since a few months back. However, the particular problems of PMOS, insulin resistance, and fatty liver, cholesterol rise started in late 2025. 

So, on 3rd Jan 2026 the first online dietitian consultation call was scheduled and in the call client expressed about managing these new problems with therapeutic diet approach. During this first consultation, Dietitian Foram Modi understood all the new problems and the new goals Isha wish to achieve in the next few months.

Clearly Disturbed Menstrual Cycle of Isha - What we know so far

Even before our dietitian for overseas people, Dt. Foram Modi asks for various blood reports of Isha to know the insulin level and also ask for the ultrasound finding, the menstrual cycle history of Isha itself was telling a powerful story. 

Please note, a healthy menstrual cycle in Indian women typically occurs around 24 to 35 days with reasonably predictable timing. However, in case of Isha, the cycle pattern was highly irregular. 

Dt. Foram Modi found that there were repeated delays which were extended beyond the normal limit. The biggest gap was of 89 days. This clearly suggested Dt. Foram Modi about chronic ovulation dysfunction. As per the Polycystic ovarian syndrome and insulin resistance: a North Indian study published by Nitin Agrawal & team from Department of biochemistry, Subharti Medical College, Meerut, Uttarpradesh, PCOS is the most common cause of chronic anovulation and highlighted the strong association between PCOS and insulin resistance.

And this was not a one time delayed period, but we found it is a recurring pattern of disrupted hormonal dysfunction. Dt. Foram Modi clearly explained to Isha that these disturbed MCs symptoms are commonly seen in women with PMOS with severe insulin resistance

Pathology Tests Results Before Starting the Diet Plan

As a part of every NRI client journey at our nutrition clinic in Mumbai, Makes You Eat by Dt. Foram Modi, we always ask clients to get the latest set of pathology blood reports for PMOS, reports of Gut health tests before a diet can be designed. 

So the first set of baseline pathology report were conducted on 5th January 2026 and we asked to perform sonography of abdomen, lipid profile, liver profile and various other parameter tests.

The fasting insulin level of 43.8 indicated severe insulin resistance. And it is a primary driver behind weight gain, cravings, delayed ovulation, irregular periods, fatty liver, and future diabetes risk.

  • Total Cholesterol: 227 mg/dL
  • LDL Cholesterol: 142 mg/dL
  • Triglycerides: 176 mg/dL

The elevated liver enzymes suggested ongoing liver stress and were consistent with the fatty liver findings seen on ultrasound.

  • Severe Steatotic Liver Disease
  • Grade S3 Fatty Liver
  • USFF: 25.9%

Ovaries:

  • Bilateral features suggestive of PCOS/PMOS

The liver findings confirmed that this was not simply a hormonal condition. Severe fatty liver is often one of the clearest signs of long-standing metabolic dysfunction and insulin resistance.
